Consumer Privacy Policy Notice

The Firm’s Consumer Privacy Policy Notice (“Notice”) explains how we collect, use, and disclose personal information about consumers and employment applicants (“applicants”), and provides certain rights that consumers and applicants may have under consumer privacy statutes.

Definition of “Personal Information”: “Personal information” is information that identifies, relates to, or could reasonably be linked with a particular consumer or household. This information is referred to in this Notice as “Personal Data.”
Applicants: Providing personal information to the Firm is voluntary. However, if you choose to provide information, it is your obligation to provide complete and accurate information throughout recruitment, application, and employment.
The Firm does not sell Personal Information or Personal Data to third parties.
Categories of Personal Data that We Collect and Disclose.

We collect and disclose Personal Data in a variety of contexts. For example, we collect and disclose Personal Data to provide products and services, and for our employment and human resource purposes. We do not disclose Personal Data for any other purpose.
The Personal Data that we collect, use, or disclose about a specific consumer or applicant will depend on, for example, our relationship or interaction with that individual.

During the past 12 months, we have collected the following categories of Personal Data:
1. Personal unique identifiers, such as full name and federal or state issued identification numbers including Social Security number, driver’s license number, and passport number;
2. Personal information, including contact details such as telephone number and address, email address, email metadata, IP address, financial information such as account number and balance, payment card details including credit and debit card numbers, as well as medical and health insurance information;
3. Characteristics of protected classes or groups under state or federal law, such as sex or marital status;
4. Purchase information, such as products and services obtained and transaction histories;
5. Internet or online information such as browsing history, and information regarding interaction with our websites, applications, or advertisements;
6. Professional or employment-related information; and
7. Inferences based on information about an individual to create a summary about, for example, an individual’s collecting preferences and characteristics.

During the past 12 months, we have also disclosed the categories of Personal Data listed above internally for our business purposes and otherwise with the person’s consent (e.g., in set registry profiles or forum signatures). We have not, however, sold Personal Data.

Sources of Personal Data

The sources from which we collect Personal Data depend on, among other things, our relationship or interaction with a specific consumer or applicant. The information below lists the categories of sources from which we collect Personal Data in different contexts.

  • From consumers or applicants directly, or other individuals acting on their behalf, through physical (e.g., paper application), audible (e.g., phone), or electronic (e.g., website, email, text, social media) sources;
  • Public records or widely available sources, including information from the media, and other records and information that are made available by federal, state, or local government entities;
  • Outside companies or organizations that provide data to support activities such as fraud prevention, underwriting, and marketing;
  • Outside companies or organizations from whom we collect Personal Data to support human resource and workforce management activities.

Note: We may not discriminate against you for exercising your rights applicable consumer privacy statutes.
Responding to Requests
We are required to verify the identity of anyone making an Access, Deletion or Change Request, and we must deny any such request if we cannot verify the requestor’s identity. In addition, privacy and data protection laws may apply to much of the Personal Data that we collect and use. When these laws apply, Personal Data may be exempt from, or outside the scope of, a particular request. As a result, in some instances, we may decline all or part of a request related to this Personal Data.

SMS/MMS Privacy Policy
We will not share your SMS opt-in information with any third party for any reason other than deliver the specific services associated with messaging campaigns. However, we may share your personal data, including your SMS opt-in or consent status, with third parties that assist us in providing messaging services, such as platform providers, phone carriers, and other vendors involved in delivering text messages.

Terms and Conditions for SMS/MMS Communication
By opting in to receive SMS communications from The Cook Law Office (“we,” “us,” or “our”), you agree to the following terms and conditions:
Program Description: you may receive text messages from The Cook Law Office regarding the following: status updates, billing notification, delivery updates when applicable, or other matters pertaining strictly to your account or services.

● Changes to Terms- We may update these terms and conditions from time to time. Any changes will be effective when we send a new SMS message or update our terms on our website. Your continued use of the SMS service constitutes acceptance of the revised terms.
● Liability- The Cook Law Office is not responsible for any delays or failures in the delivery of SMS messages due to issues with your mobile carrier or other technical problems.
